Student Support Process
1. A Student Care and Concern Report is submitted.
2. The care team reviews the case and assigns it to a case manager (member of SOS Team).
3. The case manager (member of SOS Team) may reach out to the reporting party for additional details.
4. Outreach is initiated with the student and other relevant individuals.
5. The member of the SOS Team meets with the student to evaluate their primary needs and overall well-being.
6. Member of SOS Team identifies appropriate resources.
7. An action plan is collaboratively developed by member of the SOS Team and the student.
8. Member of SOS Team conducts follows up and reassesses the case as needed.
9. Ongoing access to support and services remains ensured for the student.
